针灸治疗女性腹泻型肠易激综合征的临床疗效观察及对血清丙二醛、总抗氧化能力的调节作用

目的：观察针灸治疗女性腹泻型肠易激综合征（IBS-D）患者的临床疗效及对血清丙二醛（MDA）、总抗氧化能力（T-AOC）的调节作用。方法：选取年10月至年1月上海中医药大学附医院及上海市针灸经络研究所收治的女性IBS-D患者86例作为研究对象，随机分为温针灸组和电针组，每组43例。2组患者均取双侧天枢、上巨虚、三阴交穴，分别行温针灸或电针治疗，每周3次，共治疗3周。观察治疗前后3组患者伯明翰症状量表、IBS症状量表、肠易激综合征患者专用生命质量量表、汉密尔顿焦虑及抑郁量表评分变化情况，以评估2组临床疗效，并检测血清MDA、T-AOC水平变化。结果：治疗后2组临床症状积分、汉密尔顿焦虑及抑郁量表评分均显著降低（P＜0.），肠易激综合征患者专用生命质量量表评分显著增加（P＜0.）。温针灸组与电针组女性IBS-D患者血清MDA水平均显著降低，血清T-AOC水平均显著升高（P＜0.01）。结论：温针灸和电针治疗女性IBS-D患者均具有较好的临床疗效，可显著改善患者的生命质量及焦虑抑郁水平，并能调节血清氧化应激相关指标MDA、T-AOC表达水平。

Abstract
Objective:Toobservetheclinicalefficacyofacupunctureandmoxibustionintreatingfemalepatientswithdiarrhea-predominantirritablebowelsyndrome(IBS-D)andexploretheregulatoryeffectonserumMDAandT-AOC.Methods:Eighty-sixfemalepatientswithIBS-DadmittedtoYueyangHospitalofIntegratedTraditionalChineseandWesternMedicineofShanghaiUniversityofTraditionalChineseMedicineandShanghaiResearchInstituteofAcupunctureandMeridianfromOctobertoJanuarywererandomlydividedintoawarmneedlemoxibustiongroup(n=43)andanelectro-acupuncture(EA)group(n=43).ThepatientsreceivedwarmneedlemoxibustiontherapyorEAtherapyatbilateral“Tianshu”(ST25),“Shangjuxu”(ST37),and“Sanyinjiao”(SP6)inbothgroups,threetimesaweek,fourweeksintotal.Theclinicalefficaciesofthetwogroupswereevaluatedaccordingtothepre-andpost-treatmentscoresofBirminghamIBSSymptomQuestionnaire,IBSSymptomSeverityScale,IrritableBowelSyndromeQualityofLifeInstrument(IBS-QOL),HamiltonDepressionRatingScale(HAMD),andHamiltonAnxietyRatingScale(HAMA),andtheserumlevelsofMDAandT-AOCweredetected.Results:Aftertreatment,thescoresofclinicalsyndromescale,HAMD,andHAMAinthetwogroupsdecreased(P0.),andthescoreofIBS-QOLincreased(P0.).ThewarmneedlemoxibustiongroupandtheEAgroupshowedreducedserumMDAlevelandincreasedserumT-AOClevelinfemalepatientswithIBS-D(P0.01).Conclusion:AcupunctureandmoxibustioncanachievegoodclinicalefficaciesinthetreatmentoffemalepatientswithIBS-D,whichcansignificantlyimprovethequalityoflifeandrelievedepressionandanxietyofpatients,andcanregulatetheexpressionlevelsofoxidativestress-relatedindicatorsMDAandT-AOCintheserum.
